Fill out this form if you have lost your Driver's Education Certificate of Completion and want to request a new one.

NOTE: This form is NOT to replace a Driver Eligibility Certificate, which has an expiration date. You have to go to the student's high school for this. Driver Education Certificates of Completion do not expire.
